Information for Veterinarians

In order for a pet owner to get financial help from the trust, an application must be submitted from the Veterinarian, along with a copy of the pet’s owner’s latest federal tax return, no more than 30 days after the date of surgery.

If you are a Veterinarian and have worked with Shadow’s Veterinarian Relief before and have a password, you can log-in using the link at the top right side of the website header, or click here. Once logged in you can fill out and submit the information using the online form.
